Form 406A Report

The Form 406A report is a close copy of the report required by the FTA. You can submit this printout directly or transcribe the data from this report into the document.

The following graphic shows the parameters of the report:

Agency (If applicable) Some transit organizations that offer continuous transit service across several geographical areas consist of several units identified as agencies.
Signup Name Sign-up periods
Division Division ID and Division name
Lines

Only the Line IDs and Line names of the selected sign-up period are shown in the list.

From Date *Start date of the date range covered in the report.
To Date *End date of the date range covered in the report.
TransitID Government-assigned ID of the transit site.

Fiscal Year End

End date of the transit site's one-year or 12-month accounting period.
From Time Start time of the time period of each day covered in the report.
To Time

End time of the time period of each day covered in the report.

Date Updated Date when the data was updated.
Export as

Report format options are: PDF, CSV, Excel (spreadsheet), RTF, and Word (word processor). Set to "PDF" by default.

Note: * If the dates that you specified in the From Date and To Date fields fall within more than one sign-up period, the SignUpList dialog box appears. This may occur if your site uses different sign-up periods for different modes or divisions.
