| Provider | Choose the provider used for this trip. |
| Mobility Aid | Select the mobility aids to be used for this trip. |
| Funding Source | Select the funding source to be used for this trip. |
| Group Name | Type the group name the client is a member of, if applicable. |
| Elig. Condition | Click the eligibility condition that applies to the client.
When an eligibility condition is in effect, all rules applying to it determine whether or not the client is eligible for funding. |
| ParaService | Select the paraservice that applies to the client. |
| Mode of Transp. | Choose the mode of transportation to be used on this trip. |
